  • This plan is not insurance and is not a qualified health plan under the Affordable Care Act.
  • This plan includes a 30-day money-back guarantee. Members can receive a full refund if benefits that have not been utilized. If benefits have been utilized (i.e., the member received a cleaning, exam, and x-ray), the member may receive a refund equivalent to a pro rata portion of their purchase. Refunds after 30 days will be at the discretion of our practice.

  • Plan payments must be current for members to receive services and discounts.
  • Any treatment not included in the plan must be paid for at the completion of the service.
  • This plan cannot be combined with insurance or any other offers/discounts.
  • The plan does not cover procedures completed by specialists outside of our practice.
  • The plan covers members for one calendar year and includes an auto-renewal feature. You can opt out of this renewal at any time on our Member Portal. Members will receive a reminder of their auto-renewal 45 days before their renewal date.
